2016 AUCTION ITEM DONATION FORM

Thanks so much for considering donating an item for the 2016 Caboholic Auction! We ask that all donations be delivered The Caboholic Store prior to February 6th. If this is not possible, please note below and we'll make special arrangements.

In order to keep records more accurately, we ask that you submit the information and details on the item you are donating in the form below.

Once, we've received the information, we would appreciate any photos you have as well. These can be emailed to maddie@caboholics.com.

Please note: Although all proceeds from the auction do go to local charities, Caboholics is not a registered charity and cannot provide you with a tax deductible receipt.
